Chuck used the elements of dark and light to create contrast, thus focus and balance. The background is dark, while the moon provides a little slice of blue. The face, hand and tongue provide the light. The tongue in particular has light coming from it, as well as side of the face and the top of the hand.
The hand looks like it is closer to us because it is closer to the bottom of the painting. The scale he created makes the hand much bigger in proportion than not. The hand is bigger than her sillhouette. He also incorporated a limited palette. Flesh tones dominate the painting, with the exception of the half blue moon at the top. I don't see a a unity per se. Instead I see the proportion could be altered and the painting would not diminish the quality.
As far as directional forces what I see is two circular patterns. One with the hand, the other with the moon. The face itself is somewhat rounded, but is slightly inconsistent which draws the attention away from it and onto the extended tongue, and hand.
